POSITION SUMMARY: The g oal of the Nurse Navigator LPN is to empower patients in the post-acute selection process as well as create a seamless transition of care in alignment with the core values of LifeBridge Health.
RESPONSIBILITIES:
The Nurse Navigator LPN is responsible for educating patients with a recommendation for a skilled nursing or rehabilitation on their post-acute options. This teaching will include a discussion of quality analytics for skilled nursing facilities. The Nurse Navigator LPN must possess critical thinking skills and the ability to anticipate barriers to discharge. The Nurse Navigator LPN must be comfortable collaborating with an interdisciplinary team. There will be ongoing communication with patients and families, post-acute care facilities, and the interdisciplinary team. The Nurse Navigator LPN must be able to multi-task, manage numerous priorities, and remain productivity while managing interruptions. Tasks Review all patients with recommendation for acute care discharge to destinations other than their home setting. Specific focus on patients with a discharge plan to acute or sub-acute level of care.
